How do I use custom questions?

Custom Questions are a great way to ask questions that aren’t covered in the Questionnaire module. You are able to create any question you would like. They allow you to indicate correct and incorrect answer choices, as well as upload a media option to each option.

 

Go to the Questionnaire Module

As a facilitator:

  1. From the dashboard, click the “Patients” menu option at the top of the page
  2. Click the pencil button next to the patient you would like to enter information for
  3. Click on the “Questionnaire” tab

As a teacher:

  1. From the dashboard, click the “Students” menu option at the top of the page
  2. Click the pencil button next to the student you would like to enter information for
  3. Click on the “Questionnaire” tab

As a family member:

  1. From the dashboard, click the “Patient Profile” or “Student Profile” menu option at the top of the page
  2. Click on the “Questionnaire” tab

 

Add a New Custom Section

  1. Click the “+ New” button at the top of the Sections box

2. Select “Section”

3. A new blank section will appear at the bottom of the Sections list

4. Click the box and type in your custom section name

 

Add a New Custom Question to an Existing Section

  1. Click the “+ New” button at the top of the Sections list
  2. Select “Question”
  3. A new question will be added to the section where you can type in your customer question prompt. By default, 3 answer boxes are shown to indicate 1 correct answer and 2 incorrect answers:
  4. To add more answer choices, click the ellipses in the corner of the question’s box and select “add an answer option”

Delete a Custom Question

  1. Click the ellipses on your chosen question and then select “Delete”

 

Rename a Custom Section

  1. Click the “Edit” option above the custom section’s name you want to rename
  2. Set a new name for the section, then click any blank area on the page to save

 

Archive a Custom Section

  1. Click the “Archive” option above the custom section’s name you want to hide from the Sections list on the Questionnaire and on the Therapy Session Dashboard

 

Restore an Archived Custom Section

  1. Click the “Display archived sections?” link towards the top of the Sections list
  2. Click the “Restore” option above the custom section’s name you want to appear again in the Sections list on the Questionnaire and on the Therapy Session Dashboard

 

Delete a Custom Section

  1. Click the “Delete” option above the custom section’s name you want to permanently delete
  2. When prompted, click “OK” to confirm
